This site provides authorised Solicitors and members of the public with secure access to digital documents issued in response to a completed Subject Access Request (SAR) by a Hospital Trust or other authorised organisation.

Access to this portal is available only via a secure, time-limited email link issued by the organisation managing your Subject Access Request. This email link is the only valid method for accessing your documents.

If you are a Solicitor or a member of the public wishing to view or download documents relating to your Subject Access Request, please check your email and follow the instructions in the access link provided. The link will take you directly to your personal Secure Access Portal, where your documents can be viewed or downloaded.

If you do not have an access link, or if your link has expired, please contact the organisation that handled your Subject Access Request and request that a new link be issued. Once received, use the link in the email to access your secure portal.
