This site provides a way for authorised Solicitors and members of the public to view the results of a completed Subject Access Request application from a particular Hospital Trust or other such organisation.

Authorisation for access is delivered via an email link from the Hospital Trust/Organisation handling your Subject Access Request application. Following that link is the only valid way to use this site and obtain access to the digital documents you seek.

If you are a Solicitor or member of the public looking to find the digital documents delivered in response to your Subject Access Request application then please check your email for a link and follow that link. The link will take you to your personal Secure Access Portal where you will be able to view or download your documents. If you don't have a link for any reason then you will need to contact the organisation that was handling your Subject Access Request application to request another one. Once the email arrives follow the link it contains and it will take you to the secure access portal specific to you and your SAR request application.
